Named Accountable GP ( posted on Oct 7, 2016 )

From the 1st of April 2015, practices are required under the GMS contract to allocate a named accountable GP to all patients (including children).

All patients who were on the practice list prior to the 31st March 2016 have since been allocated a named accountable GP. Patients have been notified of their allocated named GP via text message. If you are not aware which GP you have been allocated to, please ask a member of our reception team who will provide you with this information.

All new patients who register with the practice after the 1st of April 2016 will be allocated their named accountable GP within 21 days of registration.

The named accountable GP will take lead responsibility for the coordination of all services required under the GMS contract and ensure they are delivered to each of their patients where required (based on the clinical judgement of the named accountable GP).

Please be aware this allocation does NOT stop you from seeing any of the GPs at the practice.

In the event that a patient confirms they do not want a named accountable GP, the practice will record this information in the patient record; the requirement to allocate a named accountable GP will therefore not apply. In addition any patient who expresses a preference as to which GP they have been assigned, the practice will make reasonable efforts to accommodate this request.

Update: As at the 31st of March 2016 all patients registered at The Speedwell Practice have been allocated and informed of the associated named GP in accordance with the GMS contact.

During the period of the pilot we will be offering additional GP appointments to those being currently offered.

We hope that these measures will ensure that patients’ needs could be met, therefore reducing reliance on Walk In Centres and Hospital A&E Departments

Named GP for patients over 75 years ( posted on Jun 25, 2014 )

GP practices are required to provide all their patients aged 75 and over with the name of a GP who will have overall responsibility for their care. This is their Named GP.

We have always had a system where all our registered patients (of any age) have a Named GP and this is the name which appears on the right hand side of your prescription. This may be the GP you registered with when you first joined the surgery. It may be the GP you usually see.

Although your named GP will have overall responsibility for your care, please be reassured that you can continue to see ANY of our doctors and nurses and indeed we cannot guarantee you will be able to see your named GP whenever you want to.

However, your named GP will have overall responsibility for the care and support that The Speedwell Practice provides to you.

If you would like to know who your named GP is, you can ask at reception, ask who your "usual" GP is recorded as, look on the right hand side of your prescription or ask any member of staff you are seeing.

An exciting opportunity to update our computer has finally arrived. While this is happening there may be some disruption temporarily to the appointment system especially for the pre bookable  appointments.  We apologise in advance for this disruption. We are all learning the new system and appreciate your patience if things take a little longer.
