Medication Reviews

Repeat medications are monitored on a regular basis. Different conditions require different checks to be done. These are as follows:

 Hypertension (High Blood Pressure)

  • Blood pressure checks and urine testing
  • Certain medication may also require you to have an annual blood test

Chronic Heart Disease

  • Annual blood tests including a cholesterol check
  • Blood pressure check
  • Depending on the results of the above you may be asked to make an appointment with a doctor or Linda O’Leary
  • From time to time you may be asked about your mood 

Asthma

  • Annual face to face review with a doctor or Nurse Practitioner Linda O’Leary  to assess your symptoms and check your inhaler usage and technique

Chronic Obstructive Pulmonary Disease

  • Annual spirometry to assess your breathing
  • Face to face review with Nurse Practitioner Linda O’Leary to discuss your symptoms and check your inhaler usage and technique
  • From time to time you may be asked about your mood 

Diabetes

  • Annual eye testing
  • Review with Dr Sodha or Nurse Practitioner Karen Landi.This takes place after your blood tests amd foot check.
  • To check your blood pressure and for your foot check please book an appointment with our Health Care Assistants Moira or Frances.
  • You will be notified when your blood tests are due and then contacted to make an appointment in the Diabetic Clinic if necessary. 
  • You may be offerd an appointment in our Group Conultation Clinic
  • Annual urine testing.

Thyroid Diseases

  • Annual blood tests

Chronic Kidney Disease

  • Annual blood pressure and urine testing 

Epilepsy

  • Annual blood tests
  • Face to face review with a doctor to discuss you seizure control and general health

Depression

  • If you have depression, you may be asked to fill out a questionnaire when you see the doctor or request your medication

Mental Health

  • Annual blood tests
  • Face to face review with a doctor to discuss your general well-being and medications 

Dementia

  • Annual review with a doctor to discuss your general well-being

Contraception

  • For some forms for contraception we will need to check your blood pressure at regular intervals

Hormone Replacement Therapy

  • You may be asked to have a Well Woman check on a regular basis
